How Does it Work?
Join our Membership Program at After Hours Medical for a monthly fee and receive basic medical care for a discounted all-inclusive fee of $5 per visit. This visit includes all treatments, labs, medications, and procedures that are performed in any of our 6 clinics from 9am to 9pm 7 days a week without additional charges.

Individual Membership Fee (sign up on your own):
- $49 per person per month
- $245 maximum per family (no extra charge for 6 or more)
- Minimum of 6 month term
- Sign up online or visit any clinic at any time.

Corporate Membership Fee (through your employer):
- This is a great health benefit for your employees.
- $30 per person per month (that's only $1 per day)
- $60 per 2 persons per month
- $100 per family (3 or more with no extra charge)
- Payroll deduction (pre-tax expense)

How can I Receive the Corporate Rate?
Answer: Let your employer or HR person know about this program and call Kelly at 801-860-4831 to set up a corporate account with your employer. Then the monthly membership fee can be deducted from your payroll. Your employer may choose to pay for all of the montly fee, a part of it, or not at all.
